Inova Fairfax Medical Campus is looking for a dedicated Registered Nurse (RN) Patient Care Navigator to join the Heart Failure team. This role will be full-time Day Shift: Monday – Friday, 8:00 a.m. – 4:30 p.m. (must be flexible) | Travel required – rotating to our Oakville & Mark Center locations in Alexandria

Registered Nurse (RN) Patient Care Navigator Heart Failure Job Responsibilities:
- Acts as a liaison, advocate and point of contact to patients in need of services such as screening, diagnostic treatment, clinical research participation, support services, and follow-up/aftercare plan by helping them access the best healthcare outcomes from doctors, hospitals and others who provide their required services.
- Refers cases to the appropriate resources for assistance. Anticipates patient's needs and advocates for patient rights and appropriate utilization of services.
- Conducts comprehensive assessment of the patient's health and psychosocial needs and develops a case management plan in collaboration with the patient and their support system.
- Educates patient, family or caregiver(s), and members of the health care delivery team regarding treatment options, community resources, insurance benefits, self-care management, etc. to facilitate timely and informed decision-making throughout all phases of the continuum process.
- Manages complex cases effectively and efficiently. Creates a seamless flow through the health system to facilitate the patient's care. Collaborates with multidisciplinary teams to address patient questions and responses about the care process and treatment options.
- Develops/maintains reliable systems to document, track and monitor patient data.
- Performs other duties as assigned.
Additional Requirements:
- Work schedule: full-time Day Shift: Monday – Friday, 8:00 a.m. – 4:30 p.m. (must be flexible) | Travel required – rotating to our Oakville & Mark Center locations in Alexandria
- Education: Bachelor’s degree in nursing
- Experience: 3 years of nursing. Telemetry, ICU, CVICU, and/or heart failure experience is preferred.
- Certification: Licensed in the Commonwealth of Virginia as a Registered Nurse; BLS from the American Heart Association
